My best-in-bag recommendation recommends unequipping my offhand without replacing it with anything (another offhand or a 2H weapon). When I try to lock the offhand item and re-sim, nothing changes; i.e., it still recommends unequipping my offhand.

Click the Unlock All button and re-optimise, that will clear things up.
Hi, following up to provide more details. I tried using the “Unlock All” option as Cluey advised but that did not solve my problem. In my screenshot I had my gearing preference set to “No Azerite - Haste”. When I cleared this preference and re-simmed, the simulator correctly suggested I wield a 2H weapon (no more uneuipping offhand bug). Then, when I went into the options to select a preference, “No Azerite - Haste” was no longer an option at any tier. I guess as my gear changed, “No Azerite - Haste” was no longer a viable option, but I was never notified or prompted to change this.
I’ll take a look at that… in theory if one of the options becomes no longer available (because it is too low score or is too similar to the base best-in-bags result) it should just ignore/remove the preference automatically… guess something didn’t work there.
