BiB will often recommend the same item for multiple specs, Like Prot/Ret/Holy. This can often mean that Azerite powers, enchants, and gems will benefit one spec, but not be optimal for another.

Is there a way with BiB to force unique items for each spec?

It will only do that if your other items wouldn’t be better than just using that item with off-spec traits.
