Why am I getting suggestions for multiple int+stam and sp+mp5 gems? I even tried to exclude them and still they are suggested.

You have your restoration spec set as a higher priority, so the optimizer is doing restoration first, locking the gems into that gear, and then when it looks at elemental it is not free to swap the gems. Move elemental to the top of the list to see what the optimizer prefers for elemental, or, get duplicates of all your items
