Started using AMR for tanking, I have recently levelled a new tank and I am trying to sim it - it is saying I have reached my toughness threshold when I clearly haven’t - doesn’t matter what I set the strategy to?

If your gear level is significantly lower than current content, we more or less disable the toughness threshold and go for a mostly dps-oriented build. Once your item level gets up a bit higher, then it’ll go more aggressively for toughness. I’d have to look up where it kicks in… but I’d guess around ilvl420 or so.
