AMR is telling me my best in slot legs are Silver-Trimmed Breeches (ilvl 395) which i have equipped. But current mythic raid tier is 415 ilvl. How is it possible that no other pants are better than 395? Trying to understand why my max ilvl BIS would only be 410 for mythic raid healing.

The reason it values the 395 legs so high it due to the leech they have and since you don’t have any other leech item it equips them. BUT if you check all your legs (by clicking them) you get the following:
What that means is that the 395 legs with leech and the 415 legs are pretty much the same (0.04% is negligible) so go with what ever you feel its better. Both items are the same HPS wise.
